Release Form

I hereby release and indemnify The BASIC Fund from any and all claims and liabilities arising from my child’s participation in this program.    I understand and agree that The BASIC Fund may request any information relevant to this registration from an outside source, including another program or institution.  I hereby permit The BASIC Fund to obtain, from my child(ren)’s current school as well as any schools my child(ren) may attend in the future, school records, including grades and the results of any standardized tests.  I also permit The BASIC Fund to use my child’s name and picture or video in its brochures and other materials, including those materials which may be requested from time to time by partner and donor organizations, to represent The BASIC Fund.  I also give permission for the school to provide The BASIC Fund all required documentation as part of my application and renewal process.  I hereby release and indemnify any school providing requested information to The BASIC Fund on my behalf, of and from any liability to me arising from or related to the provision of requested reports, materials or information.  I understand that if awarded and I accept this scholarship from The BASIC Fund I will not also receive funding from Guardsmen, FACE or any outside source that has not been approved by The BASIC Fund.  I understand that my The BASIC Fund scholarship will be renewable, provided that I continue to meet the participation requirements, until my child graduates 8th grade.

I agree to all terms of this Registration Agreement & Release Form. I understand that The BASIC Fund reserves the right to cancel my scholarship if information I provide is deemed to be false, or if any of the above terms and conditions are not complied with.

Por lo presente, libero e indemnizaré al BASIC Fund de cualquier y de todas y cada una de los reclamos y responsabilidades que surjan en la participación de mi hijo en este programa. Entiendo y acepto que el BASIC Fund puede solicitar cualquier información pertinente a este registro de organizaciones externas, incluyendo otro programa o institución. Por lo presente autorizo el BASIC Fund para obtener de la escuela actual o cualquier escuela que asistan mi(s) hijo(s) en el futuro, los registros de la escuela, incluyendo sus calificaciones y los resultados de las pruebas estandarizadas. También permito el BASIC Fund utilizar el nombre y la foto de mi hijo en sus folletos, videos y otros materiales que pueden ser solicitados de vez en cuando por organizaciones asociadas y donantes, para representar el BASIC Fund. También doy permiso que la escuela proporcione al BASIC Fund toda la documentación requerida como parte de mi proceso de solicitud y renovación. Por lo presente, libero e indemnizaré a cualquier escuela que proporcione información solicitada al BASIC Fund en mi nombre, de y de cualquier responsabilidad hacia mí que surja de o este relacionada con la provisión de informes, materiales o información solicitados. Entiendo que si se concede y acepto esta beca del BASIC Fund no voy a recibir financiación del Guardsmen, FACE o cualquier otra fuente que no sea aprobada por The BASIC Fund. Entiendo que la beca del BASIC Fund será renovable, siempre y cuando mi familia continúe a cumplir con los requisitos para participar, hasta que mi hijo gradué el octavo grado.

Declaro estar de acuerdo con todos los términos en este Formulario de Acuerdo de Registro y Autorización. Entiendo que el BASIC Fund se reserva el derecho de cancelar mi beca si la información que proporciono se considera falsa, o si alguno de los términos y condiciones anteriores no se ha cumplido.
